Buyer name
Institution name
E-mail address, fax and/or telephone number
Billing address
Shipping address
Date needed or shipping method desired
Also include payment information:
• IVP account number
• Credit card account information
(card holder's name, account number and expiration date)
• Personal/institutional check
(in US dollors ($) drawn on a US bank)
